Florida Fertility Institute is continuing to follow all CDC guidelines for patient and staff safety, and we are open for scheduled appointments.

Patients are asked to wear masks or cloth face coverings when in the office. We also ask that children do not attend appointments with you at this time.

New Patients

Please arrive 15 minutes prior to your scheduled appointment time.

Payment Policy

Payment is due at the time of service and includes, but is not limited to, self-pay visits, copays, co-insurance and deductibles.  Should you have any questions regarding your financial responsibility, please call (727) 796-7705.

Payment Methods Accepted

Payments may be made by cash, personal check or credit card.  Please note that a 2% merchant fee will be added to all credit card transactions.  Personal checks cannot be accepted as payment towards surgery.  Instead, a cashier’s check/money order made payable to “Florida Fertility Institute” may be substituted.

What to Bring

Be sure to bring a photo ID and insurance card, if applicable, to your visit.  Depending on your insurance, you may also require a referral or authorization.

Insurance

Every insurance plan is unique and coverage is dependent upon each individual’s policy’s terms, benefits and exclusions.  You should contact the member services department listed on your insurance card for a full explanation of benefits prior to your visit.  You may provide them our facility tax ID #593585336 for benefits specific to our practice.

Cancellations & Reschedules

We kindly ask that you give 24 hours’ notice should you need to cancel or reschedule your appointment.
