The Florida Fertility Institute is a state-of-the-art facility for diagnosing and treating infertility problems. We receive an overwhelming number of emails regarding the affordable tubal reversal procedure we perform here in the Tampa Bay area for women from around the country. It is done as an out patient procedure, via mini-laparotomy. The incision length is approximately six to seven centimeters. Only Reproductive Endocrinologists do the reversals.
In the appropriately selected patient population, a tubal reversal is effective, with excellent pregnancy rates; has a fast recovery time, very few complications, and is well tolerated. Once you have made an appointment, we ask that you have all your relevant medical records transferred to the office where you will be seen. Our staff will assist you by explaining how to have these records transferred from another doctor's office.
All of your medical records, including consultations and results of any tests performed, are strictly confidential. They will only be released with your written authorization.
The fees you are charged for services and procedures reflect the quality of care you receive, as well as the high level of training and specialization of our physicians, nurse practitioners and entire staff. We will be happy to explain these fees to you prior to treatment, and to answer any questions you may have concerning charges.
Insurance coverage and reimbursement issues for the evaluation and treatment of infertility may be complex. There are many variables in determining what reimbursement insurance companies will provide.
It is important that you understand your plan -- what it covers and what it does not. Make certain that all of you decisions in your infertility treatment process are based on your full understanding of the nature and scope of the related clinical, psychological and financial elements.
As a patient, you are responsible for all charges associated with consultations, diagnosis, testing and treatment. Payment is expected at the time of service for all office visits.
If you are a member of a prepaid health plan, please provide us with specific information at your first visit. In addition, our staff will file your insurance forms for surgical procedures. We will also try to assist you in obtaining your full benefits and payments from insurance companies. However, insurance coverage is based on a contract between you and the insurance company, and you are responsible for your medical bills regardless of the amount your insurance company pays.
